저강도 운동이 고지방식이로 유도된 비만 쥐의 지방세포에서 염증반응 및 세포사멸에 미치는 영향

Abstract

PURPOSE

The goal of this study is to demonstrate whether low intensity exercise has an effect on the level of inflammatory response and apoptosis in rats with obesity induced by a high-fat diet.

METHODS

A high fat diet (60% fat, 20% carbohydrate, and 20% protein) was treated to a subset of experimental rats for 35 weeks to induce obesity before exercise intervention. The rats were randomly assigned into 3 groups: normal control (ND-CON, n = 7), High-Fat Diet control (HFD-CON, n=7) and High-Fat Diet+Exercise (HFD-EXE, n=7).The low intensity exercise program was performed for 10 weeks with treadmill (30 min/day, 5 days/week). After treadmill exercise, body weight, total fat mass, HOMA-IR and OGTT were analyzed. Also TLR-4 signaling pathway related proteins, inflammation related factors and apoptosis were analyzed using Western blot method.

RESULTS

Low intensity exercise with treadmill reduces body weight and body fat of obese rats that were induced by high fat diet, and improves insulin resistance and blood glucose recovery ability. In addition, low intensity exercise inhibits the activity of NF-κB by reducing TLR4 protein, which regulates inflammation and immunity. This has shown to reduce the inflammatory cytokines IL-1β and TNF-α, Bax and Caspase3 proteins associated with apoptosis, and improve Bcl2 protein.

CONCLUSIONS

In conclusion, low intensity exercise reduces the level of TLR4 protein that regulates immune and inflammatory responses in adipocytes of obese rats induced by high fat diet. This inhibits the activity of inflammatory cytokines and apoptosis-related proteins, which will be effective in the prevention and treatment of obesity.

서 론

최근 에너지의 균형을 깨트리는 고열량의 식습관과 부족한 신체활동으로 인한 과체중 및 비만 인구가 급격히 증가하고 있다. 비만은 체내에 체지방이 과도하게 축적된 상태로 세계보건기구(WHO)에서 건강위험인자로 분류되어 염증반응을 비롯한 당뇨병, 고혈압, 고지혈증, 대사증후군과 같은 여러 질환과 밀접한 관련이 있다[1-3]. 지방조직은 단순히 에너지를 저장하는 기능으로 알려져 있었지만 많은 연구들에 의해 염증성 사이토카인을 분비하는 내분비기관으로 인식이 바뀌고 있다[4-6]. 또한, 지방조직은 지방세포(adipocytes), 섬유아세포(fibroblasts), 대식세포(macrophage) 등을 포함하는데 특히 대식세포는 감염된 병원체에 민감하여 다양한 사이토카인을 분비하기 때문에 면역반응에 중요한 역할을 한다[7]. 비만한 개체들의 지방조직에서 염증성 사이토카인의 생성 및 증가는 항염증성 사이토카인들과의 불균형을 초래하여 면역기능을 저하시킬 뿐만 아니라 혈당기능의 저하, 인슐린 저항성 증가, 그리고 근육량을 감소시킨다[8-9]. 이처럼 비만은 전신성 염증상태로 체내의 조직에 지속적인 염증을 발생시킨다[10]. 그러나 비만한 개체의 지방세포에서 발생하는 염증 반응과 관련된 proinflammatory cytokine은 생성 조절 기전에 대해서 정확히 알려지지 않은 상태이다[11]. 최근 많은 연구들이 제시하고 있는 기전은 지방세포에서 Toll-like receptor (TLR)의 신호 전달 기전을 보고하고 있다[12-13].
Toll-like receptors (TLRs)는 1997년에 포유동물에서 염증 반응에 관여하는 것이 발견되었고[15], 병원체를 인식하는 데 필수적인 역할을 하고 있다[16]. 현재까지 TLRs는 포유류 동종수용체로서 사람에게 10가지, 쥐에서 12가지가 연구되었다. TLRs는 막 횡단 단백질로 항원인식수용체(phthogen-sensing receptor)가 외부항원을 인지하며 신호전달이 시작된다. 이 중 TLR4는 가장 많이 연구된 수용체로 그람 음성 세균의 세포벽에 있는 lipopolysaccharide (LPS)를 인지하여 활성화된다[17,18]. TLR4는 Myeloid differentiation primary response gene 88 (MyD88)에 의존적 신호전달 체계로 활성화된다[19]. 이를 통해 면역과 염증 신호를 조절하는 Nuclear factor-kappa B (NF-κB)를 활성화시킨다[20,21]. NF-κB는 inhibitor κB (IκB)와 결합하여 비활성 상태로 존재하지만 TLR4의 신호전달에 의해 IκB가 인산화되면서 NF-κB가 활성화되어 핵 내로 이동을 하여 염증성 사이토카인인 TNF-α, IL-1β를 유발하고[19], 세포사멸과 관련된 caspase3, Bax, Bcl2를 유발한다[22-23].
Cani et al. [24]의 연구에서는 4주간의 고지방식이로 유도된 비만한 생쥐의 지방세포에서 TLR4를 활성화시켜 염증 반응의 유발을 보고하였고, Suh et al. [25]의 연구에서는 체지방은 많고, 제지방이 적은 여성들을 대상으로 TNF-α와 IL-6를 분석한 결과 체지방률이 높은 여성들에게서 염증성 사이토카인이 더 높은 수치를 나타내는 것을 확인하였다고 보고하였다. 비만은 전신성 염증상태로 간주되어 각 장기와 조직에 악영향을 초래하는 것으로 보여진다[10]. Poret et al. [26]의 연구에서는 7주간 고지방식이와 저지방식이를 실시한 동물에서 지방조직에서 염증성 사이토카인인 IL-1β와 TNF-α가 고지방식이를 실시한 동물에서 유의하게 증가하였다.
비만으로 인하여 유도된 염증성 사이토카인은 지방조직을 비롯하여 많은 조직에서 연구되고 있지만, 대다수가 약물로 처치한 연구이다. 최근 보고된 연구에 따르면 Kawanishi et al. [27]의 연구에서 동물을 대상으로 16주간의 트레드밀 운동(12-20 m/min for 60 min/day)을 실시한 결과 지방조직 내에서 TLR4의 신호전달기전의 감소를 확인하였다. 또한 Li et al. [28]은 12주간 고지방식이로 유도된 동물에서 TLR-2, TLR-4의 발현이 증가되었지만, 8주간의 트레드밀 운동(18 m/min for 60 min/day)을 통해 감소되었다고 보고하였다. Zhang et al. [29]의 연구에서는 10주간의 고지방식이를 실시한 동물들을 대상으로 13주간의 수영훈련(60 min/day, 5 day/weeks)을 실시한 결과 체중과 혈당 농도가 감소하였고, 지방조직에서 염증과 관련된 LPS, IL-6, TNF-α가 감소하였다. 그러나, Rosa et al. [14]의 연구에서는 급성 운동은 동물의 지방조직 내 염증성 사이토카인을 증가시켰다고 보고하였다. 이와 같이 운동 강도와 시간에 따라 염증반응 지표에 대한 연구를 살펴보면 장시간의 운동과 단시간 고강도 운동에서는 TNF-α와 IL-6 등이 급격히 상승한다고 보고하고 있다[30-31].
지금까지의 선행연구를 살펴보면 비만한 개체들을 대상으로 염증반응과 세포사멸에 관련된 연구를 진행중이기는 하나 운동과 관련하여 TLR4 관련 신호전달기전 연구가 부족한 실정이다. 또한, 운동 종류, 강도와 시간에 따라 염증반응과 세포사멸에 대한 효과가 불분명하다.
따라서 본 연구는 고지방식이로 유도된 비만 쥐의 지방세포에서 10주간의 저강도의 운동이 TLR4 신호전달기전에 따른 염증반응과 세포 사멸에 미치는 영향을 분석하는 데 목적이 있다.

연구 방법

1. 실험 동물

실험동물은 8주령 된 Sprague-Dawley계 수컷 쥐를 대상으로, 집단은 정상식이 비교집단(Normal Diet-Control, ND-CON; n=7), 고지방식이 비교집단(High Fat Diet-Control, HFD-CON; n=7), 고지방식이+운동집단(High Fat Diet-Exercise, HFD-EXE; n=7)으로 구분하였다. 실험동은 K대학교 동물사육실(온도 22±2℃, 습도 50 ±5%, 명암주기 12시간)에서 사육하였다. 실험동물은 비만을 유도하기 위해 35주간 중앙실험동물에서 구입한 탄수화물 20%, 지방 60%, 단백질 20%로 구성된 고지방사료(D12492)를 공급하였다. 운동을 실시하는 10주 동안에도 HFD-CON 집단과 HFD-EXE 집단에 고지방식이를 공급하였고, ND-CON 집단은 정상식이를 공급하였다. 모든 식이와 물은 자유 공급하였다.

2. 연구절차

1) 운동 프로그램

HFD-EXE 집단은 소동물용 DJ2-242, Daejong Instrument Industry Co., Seoul, Korea 장비를 이용하여 저강도 운동을 실시하였다. 사전 적응훈련(경사도 0%, 운동시작 5분 2 m/min, 다음 5분 5 m/min, 마지막 20분 8 m/min)은 1주간 실시하였다. 사전 적응훈련을 마친 후 본 운동은 주 5일, 10주간 실시하였다. 본 운동의 운동 방법은 경사도를 0%로 고정한 후에 운동시작 5분 동안 8 m/min, 다음 5분은 11 m/min, 마지막 20분은 14 m/min 속도로 Cho et al. [32], Choi et al. [33]과 Kim et al. [34]의 운동프로그램을 바탕으로 실시하였다.

2) 체중(Body weight) 및 경구당부하검사(oral glucose tolerance test, OGTT)

체중은 실험실용 저울로 고지방식이 섭취 시작 전, 고지방식이 섭취 25주 후인 운동 시작 전, 10주간의 운동 후에 측정하였다. 내당검사는 10주간 트레드밀 운동수행 후 12시간이 지난 다음 공복 상태에서 쥐꼬리로부터 혈당을 측정한 뒤, 3차 증류수 1 mL 글루코스 0.3 mg을 혼합하여 30% 수준의 글루코스 농도로 만든 후 kg당 1 mL 섭취시킨 후 쥐 꼬리로부터 총 5회(안정 시, 30분, 60분, 90분, 120분)에 각각 혈액을 채혈하여 혈당 측정기(Gluco-Card II, Daichi Kagaku. Co., Kyoto, Japan)을 이용하여 혈당을 측정하였다.

3) HOMA‐IR

실험종료 후 실험동물을 12시간 절식을 시킨 후 동물용 챔버를 이용하여 CO2가스를 흡입시켜 마취한 다음 복부를 절개하고 후대정맥에서 5 mL의 혈액을 채혈한 후 원심분리기(Beckman, USA)를 이용하여 3,000 rpm으로 30분 동안 원심분리한 후 상층액인 혈청을 스포이드를 이용해 다른 튜브에 옮긴 뒤 다음 분석 시까지 -70℃에서 보관하였다. 포도당, 인슐린의 분석은 (주)녹십자 임상검사소에 의뢰하였다. 인슐린 저항성 지표인 HOMA-IR은 다음의 공식에 의해 산출하였다.
HOMA-IR=[fasting insulin (μU/mL)×fasting glucose (mmol/L)/22.5]

4) 지방 조직 적출

10주간 저강도 운동과 운동기능검사를 수행한 후, 동물용 챔버를 이용하여 CO2가스를 흡입시켜 마취한 후, 복부 내장 지방 조직을 적출하여 분석 시까지 -80℃ 초저온냉동고(Deep Freezer, SANYO, Japan)에서 보관하였다.

5) Western Blot

단백질을 추출하기 위해 막자사발과 액화질소를 이용하여 함께 파쇄하고 Protein Extaction Solution-RIPA (ELPIS biotech, Korea)와 혼합하여 균질화 시킨 뒤 원심분리기(Micro-centrifuge, 5417R, Eppendorf)를 이용하여 원심분리(14,000 rpm, 30분)한 후 상층액을 얻었다. 각 단백질의 분자량에 따라 10%와 12%의 separating gel (3DW, 30% acrylamide: bisacrylamide, 1.5 M tris pH 8.8, 10% SDS, TEMED, 10% Ammonium persulfate)과 5% stacking gel (3DW, 30% acrylamide: bisacrylamide, 1 M tris pH 6.8, 10% SDS, TEMED, 10% Ammonium persulfate)을 SDS-PAGE에 사용하였다. 원심분리하여 얻은 상층액과 2X Sample loading buffer (60 mM tris pH 6.8, 25% glycerol, 2% SDS, 14.4 mM 2-mercaptoethanol, 0.1% Bromophenol blue)를 1:1 비율로 잘 혼합한 후 95℃에서 10분간 끓여 단백질을 변성시킨 후 10분간 ice에서 cooling하고, spin down하였다. Protein size marker (ELPIS biotech, Korea)와 함께 각 샘플을 Mini-Protein II dual-slab apparatus (Bio-Rad, CA, USA)에 준비된 stacking gel well에 각 샘플을 분주하고 80 volt에서 바닥에 내려올 때까지 전기 영동하였다. Polyvinylidine difluoride (PVDF) membrane을 MeOH에 적셔 activation 시킨 후에 Transfer buffer (190 mM glycine, 50 mM Trisbase, 0.05% SDS, 20% methanol)로 washing 한 후에 Transfer buffer에 적신 whatman 3M paper를 차례로 겹쳐 Mini trans-bolt cell (Bio-Rad, CA, USA)에 장치한 후 200 mA로 90분 전사하고 Membrane으로 증착이 끝나면 platform shaker 위에서 60분 동안 membrane을 3% BSA용액(in TBS-T: 10 mM tris-base pH 8.0, 150 mM NaCl, 0.1% Tween-20)으로 Blocking 하였다. 본 실험에 사용한 1차 항체는 Table 1과 같다. 1차 항체는 1:1,000으로 3% BSA 용액에 각각 희석시켜 12시간 동안 흔들어 주고 난 다음 0.1% TBS-T 용액으로 세척한 후 2차 항체(horseradish peroxidase-conjugated goat anti-rabbit, Invitrogen, CA, USA; horseradish peroxidase-conjugated goat anti-mouse, Santa Cruz, USA)를 Blocking 용액으로 1:5,000으로 희석시켜 90분 동안 흔들어 주고 난 다음 TBS-T 용액으로 10분씩 5차례 세척하였다. 마지막으로 LuminataTM Forte Western HRP Substrate (Millipore, USA)에 membrane을 넣고 1분간 발색시킨 membrane은 이미지 분석 시스템(Molecular Imager ChemiDoc XRS System, Bio-Rad, USA)을 이용하여 스캔한 후 Quantity One 1-D Analysis Software (Bio-Rad, USA)를 이용하여 단백질량을 산출하였다.

3. 자료처리방법

모든 자료는 윈도우용 18.0 SPSS 통계 프로그램을 이용하여 기술 통계치(mean ±SD)를 산출하였고, 집단 간 차이 분석을 위해 일원변량분석(one way ANOVA)을 실시하고 유의한 차이가 있을 경우 Bonferroni 방법을 이용하여 사후 검증을 실시하였다. 통계적 유의수준은 α=.05로 설정하였다.

연구결과

1. 집단 간 체중에 미치는 영향

저강도 운동이 고지방식이로 유도된 비만 쥐의 체중을 살펴본 결과는 Fig. 1과 같다. 고지방식이 섭취 전에는 집단 간 유의한 차이가 나타나지 않았다(F [2,18]=1.709, p =.209). 25주간 고지방식이를 섭취한 후에는 집단 간에 유의한 차이가 나타났다(F [2,18]=29.949, p =.001). NDCON 집단에 비해 HFD-CON이 유의한 차이가 나타났으며(p =.001), NFD-EXE 집단도 유의하게 증가하였다(p =.001). HFD-CON 집단과 HFD-EXE 집단은 유의한 차이는 없는 것으로 나타났다. 10주간의 저강도 운동을 실시한 후에는 집단 간에 유의한 차이가 있는 것으로 나타났다(F [2,18]=20.652, p =.001). ND-CON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났지만(p =.001), HFD-EXE 집단에서는 ND-CON 집단과 유의한 차이가 없는 것으로 나타났다(p =.470). HFD-CON 집단에 비해 HFD-EXE 집단은 유의하게 감소한 것으로 나타났다(p =.001).

2. 집단 간 체지방량과 HOMA-IR에 미치는 영향

저강도 운동이 고지방식이로 유도된 비만 쥐의 체지방량과 HOMA-IR을 살펴본 결과는 Fig. 2와 같다. 체지방량에는 집단 간 유의한 차이가 있는 것으로 나타났다(F [2,18]=32.169, p =.001). ND-CON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났으며(p =.001), HFD-EXE 집단에서는 ND-CON 집단과 유의한 차이가 나타나지 않았다(p =.111). HFD-CON 집단에 비해 HFD-EXE 집단은 유의하게 감소한 것으로 나타났다(p =.001). HOMA-IR은 집단 간 유의한 차이가 있는 것으로 나타났다(F [2,18]= 69.478, p =.001). ND-CON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났으며(p =.001), HFD-EXE 집단에서도 ND-CON 집단과 유의한 차이가 나타났다(p =.001). HFD-CON 집단에 비해 HFD-EXE 집단은 유의하게 감소한 것으로 나타났다(p =.001).

3. 집단 간 경구당부하 검사에 미치는 영향

저강도 운동이 고지방식이로 유도된 비만 쥐의 경구당부검사의 결과는 Fig. 3과 같다. 안정 시에는 집단 간 유의한 차이가 나타나지 않았다 (F [2,18]=3.251, p =.062). 30% 글루코스를 섭취한 뒤 30분 뒤에는 집단 간에 유의한 차이가 나타났다(F [2,18]=10.307, p =.001). ND-CON 집단에 비해 HFD-CON이 유의한 차이가 나타났지만(p =.001), NFD-EXE 집단에는 유의한 차이가 나타나지 않았다(p =.101). HFD-CON 집단과 HFD-EXE 집단도 유의한 차이는 없는 것으로 나타났다(p =.114). 60분 뒤에는 집단 간에 유의한 차이가 나타났다(F [2,18]=13.080, p =.001). ND-CON 집단에 비해 HFD-CON이 유의한 차이가 나타났으며(p =.001), NFD-EXE 집단에도 유의한 차이가 나타났다(p =.021). HFD-CON 집단과 HFD-EXE 집단도 유의한 차이는 없는 것으로 나타났다(p =.168). 90분 뒤에는 집단 간에 유의한 차이가 나타났다(F [2,18]= 45.053, p =.001). ND-CON 집단에 비해 HFD-CON이 유의한 차이가 나타났으며(p =.001), NFD-EXE 집단에도 유의한 차이가 나타났다(p =.001). HFD-CON 집단과 HFD-EXE 집단도 유의한 차이가 나타났다(p =.001). 120분 뒤에는 집단 간에 유의한 차이가 나타났다(F [2,18]= 64.954, p =.001). ND-CON 집단에 비해 HFD-CON이 유의한 차이가 나타났으며(p =.001), NFD-EXE 집단에도 유의한 차이가 나타났다(p =.001). HFD-CON 집단과 HFD-EXE 집단도 유의한 차이가 나타났다(p =.001).

4. 집단 간 지방조직에서 TLR4와 관련된 하부신호전달 단백질 발현에 미치는 영향

저강도 운동이 고지방식이로 유도된 비만 쥐의 TLR4, MyD88, NF-κB의 발현을 살펴본 결과는 Fig. 4와 같다. 먼저 TLR4의 발현은 집단 간 유의한 차이가 있는 것으로 나타났다(F [2,18]=15.249, p =.001). NDCON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났다(p =.001). HFD-EXE 집단에서는 HFD-CON 집단에 비해 유의하게 감소한 것으로 나타났다(p =.029). MyD88의 발현은 집단 간 유의한 차이가 있는 것으로 나타났다(F [2,18]=72.390, p =.001). ND-CON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났다(p =.001). HFD-EXE 집단에서는 HFD-CON 집단에 비해 유의하게 감소한 것으로 나타났다(p =.001). NF-κB의 발현은 집단 간 유의한 차이가 있는 것으로 나타났다(F [2,18]=36.347, p =.001). ND-CON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났다(p =.001). HFD-EXE 집단에서는 HFD-CON 집단에 비해 유의하게 감소한 것으로 나타났다(p =.001).

5. 집단 간 지방조직에서 염증성 사이토카인과 관련된 단백질 발현에 미치는 영향

저강도 운동이 고지방식이로 유도된 비만 쥐의 IL-1β와 TNF-α의 발현을 살펴본 결과는 Fig. 5와 같다. 먼저 IL-1β의 발현은 집단 간 유의한 차이가 있는 것으로 나타났다(F [2,18]=10.720, p =.001). ND-CON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났다(p =.001). HFD-EXE 집단에서는 HFD-CON 집단에 비해 유의하게 감소한 것으로 나타났다(p =.019). TNF-α의 발현은 집단 간 유의한 차이가 있는 것으로 나타났다(F [2,18] =132.061, p =.001). ND-CON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났다(p =.001). HFD-EXE 집단에서는 HFD-CON 집단에 비해 유의하게 감소한 것으로 나타났다(p =.001).

6. 집단 간 지방조직에서 세포사멸과 관련된 단백질 발현에 미치는 영향

저강도 운동이 고지방식이로 유도된 비만 쥐의 Bax, Bcl-2, Caspase3의 발현을 살펴본 결과는 Fig. 6과 같다. 먼저 Bax의 발현은 집단 간 유의한 차이가 있는 것으로 나타났다(F [2,18]=21.848, p =.001). ND-CON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났다(p =.001). HFD-EXE 집단에서는 HFD-CON 집단에 비해 유의하게 감소한 것으로 나타났다(p =.001). Bcl-2의 발현은 집단 간 유의한 차이가 있는 것으로 나타났다(F [2,18]=22.051, p =.001). ND-CON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났다(p =.001). HFD-EXE 집단에서는 HFD-CON 집단에 비해 유의하게 감소한 것으로 나타났다(p =.006). Caspase3의 발현은 집단 간 유의한 차이가 있는 것으로 나타났다(F [2,18]=7.711, p =.004). ND-CON 집단에 비해 HFD-CON 집단이 유의하게 증가한 것으로 나타났다(p =.005). HFD-EXE 집단에서는 HFD-CON 집단에 비해 유의하게 감소한 것으로 나타났다(p =.020).

논 의

최근 서구화된 식생활로 인하여 고지방식이 섭취 비율이 증가함에 따라 비만 인구가 증가하고 있다. 또한, 최근 연구들에 의하여 비만에 대한 인식이 변화하면서 비만과 관련된 연구가 진행되고 있다. 특히 비만은 만성적인 염증상태로 비대해진 지방세포에서 인슐린 저항성이 나타나며, 근육과 간에 점차적으로 유발하여 신체의 전체에 인슐린 저항성을 유발한다고 보고하고 있다[35]. 본 연구에서는 고지방식이를 섭취한 HFD-CON 그룹에서는 체중, 체지방량, HOMA-IR이 모두 증가하였고, 혈당회복능력을 저하시켰다. 그러나 10주간의 저강도 운동을 실시한 HFD-EXE 그룹에서는 체중, 체지방량, HOMA-IR은 모두 감소하였으며, 혈당회복능력도 향상된 것으로 나타났다. 이는 비만과 운동에 관련된 선행연구와 같은 결과를 나타내었다[36,37]. 장기간의 고지방식이로 인해 중성지방이 축적되어 세포내 glucose-fatty acid cycle이 활성화되어 포도당 산화를 억제하여 감소한 인슐린 민감도에 의해 혈당상승과 인슐린저항성을 일으킨 것으로 보여지지만[38-40], 10주간의 저강도 운동은 인슐린 민감도를 개선시켜 혈당을 조절하여 감소한 것으로 생각된다.
체내의 축적된 지방세포에서 면역과 염증반응의 지표인 TLRs가 발현되고 있는 것이 밝혀졌으며, 그 중 TLR4가 지방 조직에서 대표적으로 가장 많이 발현되어 비만과 연관되어 있는 각종 질병과 관련이 있는 것이 보고되고 있다[41-43]. 즉, TLR4의 활성은 비만, 염증반응, 인슐린 저항에 중추적인 역할을 하는 것으로 보고되고 있다[44]. TLR4는 세균성 LPS를 인지하여 활성화하여 MyD88에 의존적인 기전으로 하부 신호전달을 활성화하여[19], 궁극적으로는 염증 반응 조절에 있어 유전자의 전사를 담당하고 있는 핵 전사 인자인 NF-κB를 활성화시킨다[45-46]. NF-κB는 비만 동물 모델에서 지방세포와 근육 등에서 활성화되어 있다고 보고되고 있다. NF-κB의 활성화는 결과적으로 핵 내로 이동하여 염증반응과 세포사멸에 관련된 단백질들을 증가시키고, 활성화된 NF-κB의 핵 내로 전사되어 염증성 사이토카인인 TNF-α, IL-1β를 유발한다고 보고하고 있다[19].
본 연구 결과에서 저강도 운동이 고지방식이로 유도된 비만 쥐의 지방조직에서 TLR4와 관련된 신호전달과정에 어떠한 영향을 미치는지 확인하였다. HFD-CON 그룹은 TLR4가 ND-CON 그룹보다 유의하게 증가한 것으로 나타났다. 또한, 하부신호전달 신호인 MyD88과 NF-κB도 유의하게 증가하였다. 또한, 염증 반응관련 단백질인 TNF-α, IL-1β도 유의하게 증가하였다. TNF-α는 대부분의 세포에서 분비되는 종양괴사 인자로서 염증반응의 대표적인 지표로 사용되고 있으며[47], IL-1β 역시 염증성 사이토카인으로 모든 신호전달 경로에 나타나고 있다[48]. 이는 고지방식이로 유도된 비만한 쥐에서 체중, 체지방, 공복시 혈당, 인슐린 혈증 등의 증가와 지방세포에서의 TLR4 단백질 활성화를 확인한 Cani et al. [24]의 연구와 일치한다. 고지방식이는 체내에서 LPS 함유 미생물의 비율을 증가시켜 인슐린 저항성을 증가시키며 TLR4를 활성화한 것으로 보여진다. Song et al. [49]의 연구에서도 본 연구의 결과와 같이 비만한 쥐의 지방조직에서 TLR4의 mRNA 수준이 현저하게 증가하였고, 이로 인하여 NF-κB, TNF-α과 IL-1β이 증가함을 보고하였다. NF-κB에 의해 활성화 된 TNF-α은 인슐린 감수성을 직접적으로 손상시켜 인슐린 저항성과 염증반응을 일으키는 것으로 보고되고 있다[50]. 그러나 트레드밀을 이용한 저강도 운동을 통해 지방조직에서 HFD-EXE 그룹에서는 HFD-CON 그룹보다 TLR4를 비롯하여 MyD88과 NF-κB이 유의하게 감소한 것으로 나타났다. 또한, TNF-α, IL-1β도 유의하게 감소하였다. 비교대상은 다르지만 Choi et al. [33]의 연구에서도 지구성 운동을 통해 TLR4, NF-κB와 TNF-α가 감소하였다고 보고하였다. 추후 NF-κB는 핵을 분리하여 핵 내로 전사하여 활성된 유전자 분석이 필요하다. TLR4의 감소는 운동을 통한 인슐린 저항성을 감소시키는 기전 중에 하나라고 Francaux [51]는 보고하였다. 12주간 고지방식이로 비만 유도된 동물을 대상으로 8주간의 트레드밀운동을 실시한 결과 TLR4의 발현이 감소된 선행연구[28]와 고지방식이로 유발된 비만 쥐를 대상으로 16주간의 지구성 운동을 실시한 후 지방조직 내에서 TLR4과 TNF-α를 확인한 결과 운동을 실시한 그룹에서 현저하게 감소했다는 선행연구와 일치하는 것으로 나타났다[27]. 그러나 Rosa et al. [14]의 연구에서는 70%의 부하로 트레드밀을 이용한 급성운동한 동물에서는 TLR4, MyD88이 증가한 것을 확인하였다. 고강도의 급성 운동은 오히려 지방산에 의해 TLR4의 하부 신호전달 과정을 활성화한다고 보고되고 있다[42,52]. 위의 결과를 토대로 보면 운동과 같은 신체활동은 염증반응과 인슐린 저항성을 감소시키는 것으로 생각된다[53].
NF-κB의 활성화는 세포사멸과 관련된 Bax, Bcl2, caspase3을 유발한다고 보고되고 있다[22]. Bax는 주로 세포질에 존재하지만 지방조직의 축적으로 사립체 외막으로 이동한다. 이로 인해 Cytochrome-c를 방출하여 Caspase3을 활성화시킴으로써 세포사멸을 유발한다. 반면에 Bcl2는 세포사멸을 억제하는 단백질로 사립체 막에서 Bax 단백질의 소중합성형성(oligomerization)을 억제하여 방해한다[54-56].
본 연구 결과에서 저강도 운동이 고지방식이로 유도된 비만 쥐의 지방조직에서 세포사멸과 관련된 신호전달과정에 어떠한 영향을 미치는지 확인하였다. Caspase3, Bax, Bcl2 모두 HFD-CON 그룹은 ND-CON 그룹보다 유의하게 증가한 것으로 나타났다. 그러나 저강도의 운동을 통해 HFD-EXE 그룹에서는 HFD-CON 그룹보다 모두 유의하게 감소한 것을 확인할 수 있었다. 고지방식이를 실시한 비만 쥐의 지방조직에서 Bax, Bcl2의 증가를 확인하였으나, 유산소운동을 실시한 결과 세포사멸과 관련된 Bax와 Bcl2가 유의하게 감소를 보고한 선행연구와 일치하였다[23].
이러한 결과를 통해 저강도 운동은 비만한 개체의 불균형한 에너지대사를 개선하고, TLR4의 신호전달을 감소시켜 염증반응과 세포사멸을 억제하는 데 효과적인 것으로 확인하였다.

결 론

본 연구는 저강도 운동이 고지방식이로 유도된 비만 쥐의 지방세포에서 염증반응과 세포사멸의 발현에 미치는 영향을 규명하고자 하였다. 그 결과 저강도 운동은 지방세포 내 면역과 염증을 조절하는 TLR4의 수준을 감소하여 염증성 사이토카인과 세포사멸과 관련된 단백질의 활성을 억제하기 때문에 비만의 예방과 치료에 효과적일 것이다.
