Trans Abstract

PURPOSE

The purpose of this study was to investigate the effects of exercise and cognitive intervention program on cognitive function, sedentary behavior and health-related quality of life in mild cognitive impairment elderly women with hypertension and obesity.

METHODS

Sixteen elderly women were assigned in exercise and cognitive intervention group (n=8), and control group (n=8). The exercise and cognitive intervention program consisted of program 90 min/session, 2 days each week for 12 weeks.

RESULTS

In body composition and blood pressure, weight (p=.000), body mass index (p=.000), waist circumference (p=.003), waist hip ratio (p=.032), systolic blood pressure (p=.001), and diastolic blood pressure (p=.006) had a statistically significant. In cognitive function and sedentary behavior and health-related quality of life, K-MMSE (p=.001), sedentary time (p=.000), general health (p=.002), physical function (p=.001), role physical (p=.022), role emotional (p=.001), social function (p=.009), mental health (p=.002), and vitality (p=.011) had a statistically significant.

CONCLUSIONS

These findings suggest that exercise and cognitive intervention program may represent an effective intervention strategy for cognitive function and sedentary behavior and health-related quality of life.

서 론

통계청 자료에 따르면 우리나라는 인구의 감소와 더불어 65세 이상의 인구비율이 2018년 14.3%로 증가하여 고령화 사회로 접어들었으며[1], 최근 치매의 전 단계로 알려진 경도인지 장애(Mild Cognitive Impairment, MCI) 환자의 유병률은 22.7%, 치매 환자의 유병률은 10.28%의 증가율을 나타내고 있다[2]. 따라서 노인들의 인지기능 향상을 위한 예방적 중재의 필요성이 강조되고 있는데[3], 특히 치매의 경우 여성이 남성에 비해 2-4배가량 발병률이 높은 것으로 보고되고 있어[4], 치매와 여성 노인의 연관성이 중요할 것으로 사료된다.

Kang et al. [5]은 치매는 언어, 기억, 행동 등 뇌의 기능 부분에 손상을 주기 때문에 노화에 의해 경험하게 되는 기억력 감퇴보다 훨씬 심각한 결과가 나타나고 뇌의 기능뿐만 아니라 일상생활 등에서도 부정적인 영향을 미치는 것으로 보고하였는데, 치매가 발병하게 되면 완전한 치료가 어렵고 삶의 질을 감소시킬 뿐만 아니라, 부양가족의 경제적, 심리적 부담과 고통을 동반하고 방치할 경우 사망률이 4배까지 증가되는 것으로 보고된다[6].

한편, 우리나라의 60대 고령여성의 비만율은 42.7%로 보고되고 있으며[7], 고혈압 고령자의 비만율이 약 40%를 차지하고 있는 것으로 보고되고 있다[8]. 특히 비만은 고령자에게 있어 인지 기능을 저하시키고, 치매의 위험 인자가 되는 것으로 보고되며[9-12], 높은 혈압은 인지기능 저하와 관련이 있는 것으로 보고되고 있기 때문에[39], 치매의 예방을 위해서는 비만과 혈압의 관리가 매우 중요함을 알 수 있다. 또한 고령자에게 있어 좌식행동이 고령자의 지방 침착과도 뚜렷한 관련이 있기 때문에[13], 앉아 있는 습관의 개선이 중요할 것으로 사료된다.

고령자들에게 있어 건강관련 삶의 질은 삶의 만족도의 의미를 넘어 실생활에 제한과 문제 및 독립된 행동과 활동의 자유를 나타나는 수단으로 보고되고 있는데[14], 이 중 인지 기능의 저하는 일상 환경 속에서 일어나는 일들을 정상적으로 수행하지 못하고 독립적인 일상생활 수행 능력과 사회생활에 큰 걸림돌이 된다[15].

인지기능의 저하와 치매의 예방을 위해 다양한 중재방법이 보고되고 있다. 특히 약물치료의 경우 치매의 종류나 환자의 순응도 등을 고려해야 되며[16], 고비용과 부작용 등의 우려가 있으므로 비 약물치료를 이용한 중재방안이 효과적이라 사료된다[17].

비약물적 치료에 관한 선행연구를 살펴보면, 단일 요법의 경우 일관된 효과를 보이지 못하는 것으로 보고되고 있고[18], 단일과제보다는 이중과제 수행 시 뇌의 혈류량이 증가하고[19], MCI 고령자를 대상으로 운동과 인지 복합 프로그램을 적용한 결과 인지기능이 향상될 뿐만 아니라, 삶의 질에도 긍정적인 영향을 주는 것으로 보고된다[17,20-24].

따라서 MCI 고령자의 치매예방과 지연을 위해서는 복합적인 차원의 중재가 필요하며[25], 이중과제 형태의 복합 과제 적용이 전반적 인지기능 향상에 효과적일 것으로 사료된다[26].

그러나 치매예방에 다양한 과제를 접목한 복합적인 중재 프로그램을 수단으로 활용한 연구는 미흡한 실정이며, 연구대상이 고령자를 대상으로 한 지역사회 기반의 연구에 국한되어 있다.

따라서 본 연구는 전문의에 의해 고혈압과 MCI 진단을 받은 비만 여성 노인을 대상으로 신체활동과 인지 중재 프로그램이 인지기능과 좌식 행동 및 건강관련 삶의 질에 미치는 영향과 각 변인들 간의 상호관련성을 규명하는 데 목적이 있다.

연구 방법

1. 연구 대상

B광역시에 거주하고 있는 만 65세 이상의 여성 노인을 대상으로 체질량지수(Body Mass Index, BMI)가 아시아-태평양 기준(Asian-Pacific cutoff points) BMI≥25 kg/m2에 해당하며[27], 전문의에 의해 고혈압과 경도인지장애 진단을 받은 자로 하였다. 고혈압 약을 복용 중인 대상자의 경우 실험 기간에도 계속적으로 혈압강하제를 복용하도록 하였으며, 평상시 생활습관을 유지하도록 하였다.

대상자는 연구의 취지와 실험내용에 대해 설명하고, 자발적 참여의사를 밝힌 자로 신체활동 준비 설문지(Physical Activity Readiness Questionnaire, PAR-Q) [28]를 통해 신체활동에 제약이 없는 자로 중재 프로그램군(n= 8)과 대조군(n= 8)으로 무선 배정하였다. 본 연구는 D대학교 연구윤리심의위원회의 승인을 받아 실시하였다.

2. 연구절차

1) 신체조성 및 혈압 측정

프로그램 실시 전과 12주 후 체성분분석기(N20, AIIA Communication inc., Korea)를 이용하여 신장, 체중, 체지방률 그리고 체질량지수를 산출하였으며, 줄자(Martine식 신체계측기)를 이용하여 엉덩이와 허리둘레를 측정하였고 허리, 엉덩이 둘레비(Waist Hip Ratio, WHR)는 Waist/Hip Ratio 공식에 의해 산출하였다. 혈압은 무수은혈압계 CKE301 (Chin Kou Medical Instrument Co., Ltd., Taiwan)을 이용하여 2회 측정 후 평균 값(수축기와 이완기)을 기록하였다.

2) 인지기능 검사

Folstein et al. [29]이 개발한 Mini-Mental State Examination(MMSE)을 한국노인들이 사용할 수 있도록 번안된 한국어판 K-MMSE를 사용하였다[30]. K-MMSE는 반복적인 측정으로 인지기능의 변화를 관찰할 수 있는 설문도구로서, 세부항목은 시간지남력(5점), 장소지남력(5점), 기억등록(3점), 주의집중과 계산(5점), 기억회상(3점), 언어(8점) 및 시공간구성(1점)으로 구성되어 있다. 총점은 30점이며, 기능이 좋을수록 높은 점수를 받게 된다.

3) 좌식행동 분석

신체활동량은 가속도 센서가 부착된 가속도계(Kenz lifecorder, Tokyo, Japan)를 이용하여 12주간 측정하였다. 좌식 행동의 측정기준은 중강도부터 고강도의 신체활동(moderate to vigorous physical activity, MVPA)을 제외한 수면 시간, TV 시청, 독서 및 운전 등 모든 좌식행동을 분석하였다[31].

4) 건강 관련 삶의 질 평가

건강 관련 삶의 질 평가는 Short Form Health Survey Questionnaire (SF-36) [32]을 우리나라의 실정에 맞춰 수정 보완한 Koh et al. [33]의 도구로 측정하였다. SF-36은 36개 문항으로 이루어져 있으며, 총 점수가 높을수록 건강관련 삶의 질이 점수가 높음을 의미한다.

3. 운동과 인지 중재 프로그램

운동과 인지 중재 프로그램은 준비 운동과 정리 운동은 각각 10분씩 실시하고 본 운동 70분을 실시하여 1회 90분, 주 2회, 12주간 실시하였다. 인지과제는 4주간의 운동 적응 후 5주차부터 실시하였으며, 운동을 실시하면서 셈하기, 말하기, 끝말잇기, 박수 치기 등 다른 하나의 과제를 주어 실시하였다. 날짜 이외의 좌식 행동을 감소시키기 위해 가속도센서를 이용하여 개개인별 신체활동량을 파악하고 직접 확인시켜 주었으며, 운동 강도 설정은 1-4주는 여유 심박수의 40-50%, 5-12주는 50-60%로 설정하며, 운동 강도 설정을 위한 운동 중 심박수 측정은 무선심박수계(Polar RA400TM, Kempele, Finland)를 사용하였다. 대조군의 경우 일상생활을 유지하도록 하였다. 자세한 프로그램은 Table 1과 같다.

4. 자료처리방법

SPSS-PC 22.0 통계프로그램을 이용하였고, 모든 변인의 측정 결과를 평균(Means)과 표준편차(Standard Deviations)로 산출하고, 각각의 변인에 대한 그룹 간 차이검증은 two-way repeated ANOVA를 실시하였다. 또한 집단과 시기 간 유의성이 있을 경우 대응 표본 t-검증(paired t-test)을 실시하고, 각각의 변인들 간의 상관관계를 알아보기 위하여 Pearson의 적률 상관계수(r)를 이용하여 산출하였다. 모든 통계적 유의수준은 p<.05로 설정하였다.

연구 결과

1. 신체조성 및 혈압의 변화

운동과 인지 중재 프로그램군과 대조군의 전과 12주 후 신체조성 및 혈압의 변화는 Table 2와 같다. 체중(p =.000), 체질량지수(p =.000), 허리둘레(p =.003), 엉덩이 허리둘레 비(p =.032), SBP (p =.001), 그리고 DBP (p =.006)가 그룹과 시기 간의 상호작용에서 유의한 차이를 나타내었다.

2. 인지기능의 변화

운동과 인지 중재 프로그램군과 대조군의 전과 12주 후 인지기능의 변화는 Fig. 1과 같다. K-MMSE (p =.001)가 그룹과 시기 간의 상호작용에서 유의한 차이를 나타내었다.

3. 좌식행동의 변화

운동과 인지 중재 프로그램군과 대조군의 전과 12주 후 좌식행동의 변화는 Fig. 2와 같다. Sedentary time (p =.000)이 그룹과 시기 간의 상호작용에서 유의한 차이를 나타내었다.

4. 건강관련 삶의 질의 변화

운동과 인지 중재 프로그램군과 대조군의 전과 12주 후 건강관련 삶의 질의 변화는 Table 3과 같다. 운동과 인지 중재 프로그램 전과 12주 후 General health (p =.002), Physical function (p =.001), Role physical (p =.022), Role emotional (p =.001), Social function (p =.009), Mental health (p =.002), 그리고 Vitality (p =.011)가 그룹과 시기 간의 상호작용에서 유의한 차이를 나타내었다.

5. 상관관계

신체조성과 혈압, 인지기능, 좌식행동 및 건강 관련 삶의 질의 상관관계는 Table 4와 같다. K-MMSE는 WHR (r=-.644, p =.007)와 SBP (r=-.511, p =.042)와 유의한 부적 상관이 나타났으며, GH (r=.685, p =.003), PF (r=.551, p =.027), 그리고 VT (r=.598, p =.014)와 유의한 정적 상관이 나타났다. 한편, Sedentary behavior는 BMI (r=.663, p=.005), SBP (r=.628, p =.009)와 유의한 정적 상관이 나타났으며, GH (r=-.628, p =.009)와 MH (r=-.722, p =.002)와 유의한 부적 상관이 나타났다.

논 의

본 연구는 인지기능과 밀접한 관련이 있는 비만과 고혈압을 동반한 MCI 여성 노인을 대상으로 신체활동과 인지 중재 프로그램이 인지기능과 좌식 행동 및 건강관련 삶의 질에 미치는 영향과 각 변인들 간의 상호 관련성을 규명하고자 하였다.

비만은 만성 질환뿐만 아니라 인지기능[10,12,34]과 치매[9]에도 부정적 영향을 미친다고 보고되고 있다. 그리고 높은 엉덩이 허리 둘레비(WHR)를 가질수록 노년기에 인지 기능저하의 위험이 있는 것으로 보고된다[34,40]. 또한 Jason et al. [10]은 정상 체중을 가진 그룹에 비해 과체중과 비만군의 경우 노년기에 인지 기능 저하와 치매로의 이환율이 각 1.64배 높은 것으로 보고하여, 효과적인 비만의 관리가 치매와 인지기능 저하에 긍정적인 영향을 미칠 것으로 사료된다.

본 연구의 결과 신체조성에서 체중, 체질량지수, 허리둘레, 그리고 엉덩이 허리 둘레비가 그룹과 시기 간의 상호작용에서 유의한 개선이 나타났다.

Larsen et al. [13]은 고령자에게 있어 좌식행동이 고령자의 지방 침착과도 뚜렷한 관련이 있음을 보고하였고, 본 연구의 결과에서도 체질량지수와 좌식행동이 유의한 정적 상관이 나타났으며, 운동과 인지 중재 프로그램을 통한 좌식행동의 개선이 신체조성의 개선에도 긍정적인 영향을 미친 것으로 사료된다.

Park et al. [31]은 비만 고령여성을 대상으로 한 연구에서 좌식행동과 혈압이 정적 상관이 있음을 보고하였고, 비만 및 혈압을 예방하기 위해서는 신체활동량을 늘리는 것도 중요하지만, 좌식행동을 최소화하는 것이 중요하다고 제언하였다.

본 연구에서는 수축기와 이완기 혈압이 유의하게 개선된 것으로 나타났는데, 본 연구에서도 좌식행동과 수축기 혈압이 유의한 정적상관이 있는 것으로 나타나 좌식행동의 감소가 혈압의 개선에도 긍정적인 영향을 준 것으로 사료된다. 따라서 노인 여성에 있어 규칙적인 운동과 인지 중재 프로그램이 신체조성의 개선과 혈압의 개선에도 긍정적인 영향을 준 것으로 사료되며, 이는 인지기능과 관련이 있다고 보고한 선행연구와 비교하여 보았을 때, 의미 있는 결과라 생각된다.

Barnes et al. [35]은 인지 장애의 대표적인 원인으로 신체 활동의 부족과 노인 인구의 증가 등을 보고하였는데, 특히 MCI의 경우 인지기능의 저하와 함께 신체활동량이 저하되면서 신체적 의존성을 보이는 특징이 나타난다[36]. 성공적인 노화는 노인인구의 증가와 함께 현대사회의 화두로 거론되고 있기 때문에, 인지기능과 삶의 질의 유지 및 개선을 위한 방안이 필요하다[37].

최근 Kim et al. [26]은 경도인지장애 노인을 대상으로 운동, 인지 이중과제를 실시한 결과 뇌의 특정 기능에 작용하는 단일 과제보다는 뇌의 인지기능이 저하되는 영역을 고려하여 이중과제 형태의 복합 과제를 실시하는 것이 인지 기능을 향상시키는 데 효과적임을 보고하였다.

본 연구의 결과 K-MMSE가 그룹과 시기 간의 상호작용에서 유의한 결과가 나타났다. 이는 Kim et al. [38]의 복합 인지향상프로그램이 여성 노인의 인지기능 개선에 효과적이라는 보고와 일치하며, Kim et al. [26]이 제시한 이중과제 형태의 복합과제가 효과적임을 입증하는 결과라 사료된다. 또한 Peters et al. [39]은 고령자에게 높은 혈압은 인지기능 저하와 관련이 있는 것으로 보고하고 있으며, 높은 WHR은 인지기능과 관련이 있는 것으로 보고된다[34,40]. 본 연구는 K-MMSE가 SBP 그리고 WHR와 유의한 부적상관이 나타났는데, 이는 운동과 인지 중재 프로그램을 통한 신체조성과 혈압의 개선이 인지기능의 개선에도 긍정적인 효과를 준 것으로 사료된다.

Santos et al. [14]은 노인에게 있어 건강관련 삶의 질은 삶의 만족도를 넘어서 매우 중요한 수단으로 보고하였고, Hong et al. [37]은 24주간 장수 운동프로그램을 통해 여성 노인의 주관적 건강관련 삶의 질의 신체적, 정신적 지표가 개선되었으며, 여성 노인이 스스로 운동의 효과를 체감하고 있다는 결과로서 여성 노인의 일상생활을 긍정적으로 개선시켜 성공적 노화를 기대할 수 있음을 제언하였다.

본 연구는 건강관련 삶의 질의 지표 중 신체적 통증을 제외한 일반적 건강, 신체적 기능, 신체적 역할제한, 감정적 역할제한, 사회적 기능, 정신 건강, 활력의 항목에서 그룹과 시기 간의 상호작용에서 유의하게 개선되고, 인지 기능과 좌업 행동 그리고, 삶의 질의 지표 대부분이 유의한 상관이 나타났는데, 이러한 결과는 인지기능의 증가, 그리고 좌업행동의 감소가 삶의 질의 개선에 긍정적인 영향을 준 것으로 생각된다. 또한 주관적 삶의 질의 개선이 여성 노인에게 있어 긍정적인 영향을 줌으로써 보다 좌업 행동의 감소와 신체활동의 증가에도 기여할 수 있을 것으로 사료된다.

따라서 운동과 인지 중재 프로그램은 고혈압과 비만을 동반한 경도 인지장애 여성 노인에게 있어 신체조성 및 인지기능과 좌업 행동의 감소, 그리고 건강관련 삶의 질에 효과적인 프로그램이라 사료된다.

결 론

여성 노인에게 있어 운동과 인지 중재 프로그램은 신체조성, 인지기능 그리고 건강 관련 삶의 질에 긍정적인 영향을 주는 것으로 나타났다. 특히 좌업 행동의 개선은 여성 노인에게 있어 신체활동량의 증가와 더불어 치매예방에 효과적이라 사료된다.
