Trans Abstract

Purpose

The study aimed to examine whether handgrip strength (HGS) expressed as absolute or relative to body weight is associated with fasting glucose (FG), hemoglobin A1c (HbA1c) and the prevalence of diabetes mellitus (DM) in different age categories.

Methods

A total of 28,129 adults from the Korea National Health and Nutrition Examination Survey of 2014-2018 was analyzed. To examine the relationship between HGS and variables related to DM, participants were categorized into three groups according to their HGS (Tertile). Then, participants were further categorized into six groups according to their age. One-way ANOVA and logistic regression analyses were performed.

Results

Compared with participants in the upper tertile of absolute handgrip strength (AHGS), those in the lower tertile were older, shorter and heavier and also had higher FG and HbA1c. When age was adjusted, the prevalence of DM was 1.19 times (95% CI: 1.03-1.38) higher among men in the lowest tertile of AHGS. On the other hand, compared with participants in the highest tertile of relative handgrip strength (RHGS), those in the lowest tertile had 2.10 times (95% CI: 1.87-2.41) and 2.42 times (95% CI: 2.08-2.81) higher prevalence of DM in men and women, respectively. When the associations were examined according to age subcategories, significant associations between AHGS and the prevalence of DM were seen only in men in their 50s and 60s, but not seen in women in all age groups, with the exception of the 60s. However, significant associations between RHGS and the prevalence of DM were seen in all age subcategories.

Conclusions

We concluded that the association between HGS and the prevalence of DM was dependent on age and RHGS is a stonger measure than AHGS.

서 론

세계보건기구 (World Health Organization)의 발표에 의하면, 전세계 적으로 18세 이상 성인 중 당뇨병을 진단 받은 사람이 1980년도에 4.7%에서 2014년도에는 8.5%로 증가하였다[1]. 우리나라의 경우, 30세 이상 성인에게서 당뇨병 유병률이 2001년도 8.6%에서 2013년 13,7%로 증가하였다고 보고 되었다[2]. 당뇨병 유병률을 증가시키는 원인으로서 흡연[3,4], 노화[5,6], 비만[7], 체력 저하[8] 그리고 낮은 수준의 신체활동[9] 등이 보고 되고 있다.

체내 당 대사가 가장 활발하게 일어나는 조직 중 하나가 근육이기 때문에[10], 근육량 감소는 대사성 질환 위험을 증가시킨다[11,12]. 또한, 근력은 대상자의 근육량뿐만 아니라 운동 참여 정도를 반영하기 때문에 대사성질환의 위험 예측 요인으로 많이 사용되고 있다[13,14]. 악력은 근력을 대표하는 지표 중 하나로써[15,16], 대사성 질환의 유병률 및 발병률과 높은 관계성이 있는 것으로 보고되고 있다[1725]. 악력은 일반적으로 악력계로 측정된 수치를 절대적인 값 kg으로 표현하는 절대 악력과 이를 체중 또는 비만도 (BMI)로 나눈 상대 악력이 있다. 상대 악력은 악력과 질환의 유병률 관계에서 체중이 교란 변수 (con-founding factor)로써 미치는 영향력을 최소화할 수 있는 방법으로 사용된다[26]. 이러한 이유로 대사증후군과 같은 비만과 연관성이 높은 질환에 대해서는 대부분의 선행 연구에서 상대 악력을 이용하여 질병의 유병률을 조사했다[17,2124]. Byeon et al. [17]의 연구에서 한국 성인 남녀 1,009명을 대상으로 상대 악력과 절대 악력의 삼분위수에 따른 대사증후군 유병률을 각각 분석한 결과, 절대 악력은 대사증후군 유병률과 관계가 없는 것으로 나타났으나, 상대 악력은 상위 삼분위 그룹과 비교해 하위 삼분위 그룹의 대사증후군 유병률이 남성은 2.52배, 그리고 여성은 5.01배 더 높은 것으로 나타났다. 또한, Chun et al. [19]이 한국 65세 이상의 노인을 대상으로 악력에 따른 대사증후군의 유병률을 조사한 결과, 상대 악력은 대사증후군 유병률과 높은 상관관계가 있음을 관찰한 반면, 절대 악력은 대사증후군 유병률과 관련이 없음을 관찰하였다. 하지만, 영국 UK 바이오뱅크 연구에 따르면, 성인 356,721명을 대상으로 추적 조사를 실시한 결과, 절대 악력이 낮을수록 심혈관계 질환의 발병률 및 사망률이 증가한다고 보고하였다[20]. 이러한 선행 연구들의 상반된 결과는 질환의 특성에 따라 유병률이 상대 악력[17,19,21,23] 또는 절대 악력과 관련성이 있음을 제시한다[20].

당뇨병은 비만과 강한 관계성을 보여주는 동시에[27,28], 대사이상과 신경장애에 따른 근위축 및 근기능의 저하를 특성으로 갖는 질환으로 알려져 있다[29,30]. 또한, Park et al. [31]의 연구에 따르면 우리나라 전체 인구에 대한 당뇨병 유병률은 매년 지속적으로 증가하고 있고, 연령을 세분화하여 보았을 때는 50대부터 급속하게 증가하는 것으로 나타났다. 특히, 50대 이후에 근육량이 급격히 감소하는 것을 고려할 때에, 악력과 당뇨병 유병률 관계를 조사함에 있어 연령을 세분화하여 관계성을 분석하는 것이 필요한 것으로 사료된다[32,33]. 또한 악력의 세기와[19,34] 당뇨병 유병률[5] 모두가 연령이 증가함에 따라 변화함에도 불구하고, 악력과 당뇨병 유병률의 관계성을 연령대 별로 비교하는 연구가 매우 부족한 실정이다. 이에 본 연구에서는 국민건강영양조 사 2014-2018년 자료를 이용하여 상대 악력 및 절대 악력과 당뇨병 유병률의 관계성을 살펴보고자 하였다. 추가적으로 대상자를 10세 단위로 나누어 연령대 별로 악력과 혈당, 당화혈색소, 그리고 당뇨병 유병률의 관계성이 어떻게 나타나는지 알아보고자 하였다.

연구방법

1 연구 대상

국민건강영양조사는 보건복지부 질병관리본부에서 국민의 건강수준, 건강행태, 식품 및 영양섭취 실태를 파악하기 위해 시행하는 전국 규모의 단면 조사로, 매년 전문조사 수행 팀이 건강설문조사, 영양조사, 그리고 검진조사를 수행하고 있다 (국민건강영양조사 홈페이지, http://knhanes.cdc.go.kr/knhanes).

본 연구는 국민건강영양조사 제 6기 2,3차년도 (2014, 2015년) 및 제 7기 1, 2, 3차년도 (2016, 2017, 2018년)에 참여한 만 19세 이상 성인을 대상으로 시행하였다. 본 연구에서는 전체 대상자 중 19세 이하인 대상자 (7,889명)와 절대 악력 및 체중 변인이 없는 자 (3,181명)를 제하고, 총 28,129명 (남성 12,471명, 여성 15,658명)을 분석에 사용하였다.

2 측정 변인

1) 체구성 변인

국민건강영양조사에서 실시한 신체계측으로 신장, 체중, 허리둘레, 체질량 지수를 활용하였다. 신장과 체중은 각각 신장계 (Seca 225, GmbH&Co.KR, Germany)와 체중계 (GL-6000-20, Caskorea, Korea), 허리둘레는 인체공학적 둘레 줄자 (Seca 201, GmbH&Co.KG, Germany)를 이용하여 측정하였다. 허리둘레는 마지막 늑골의 하단과 장골 능선 상단의 중간지점을 측정하였으며, 신체 계측의 모든 측정 값은 소수점 한 자리까지 기록하였다. 체질량 지수는 체중과 신장을 활용하여 체중 (kg)/신장 (m2)으로 계산하였다.

2) 악력

국민건강영양조사에 참여한 대상자 중 팔, 손, 엄지손가락의 결손 및 골절, 손의 마비, 손과 손목 깁스 또는 붕대, 최근 3개월 내 손목 수술 및 관절염 병력, 최근 7일 이내 손의 통증, 쑤심, 뻣뻣함 경험 또는 악화 등을 겪은 대상자는 측정에서 제외되었다. 악력은 디지털 악력계 (TTK 5401, Takei Scientific Instruments Co. Ltd, Japan)를 사용하여 측정하였다. 악력계는 대상자의 손가락 두 번째 마디가 90º로 굴곡된 높이로 손잡이를 조절하였다. 측정은 양 발을 어깨 넓이로 벌리고 선 채로 팔꿈치나 손목이 구부려 지지 않게 하여 실시하였다. 양 손을 교대로 1분 간격의 휴식 시간을 갖고 왼손과 오른손 각각 3회 실시하였다. 본 연구에서는 총 6회 측정값 중 최대값을 절대악력으로 사용하였으 며, 최대값을 체중 (kg)으로 나눈 값을 상대악력으로 사용하였다.

3) 인구통계학적 변인

인구통계학적 변인으로 건강행태 요인과 사회경제적 요인이 사용되었다. 음주 빈도 (없음, 1달에 2회 이하, 2-4회, 4회 이상), 흡연 유무 (비흡연, 과거, 현재), 교육수준 (초등, 중등, 고등, 대학교 이상), 가구 소득 (하, 중하, 중상, 상), 그리고 근력 운동 실시 여부 (전혀 하지 않음, 주 1회 이상)를 각 기준에 따라 구분하였다. 동반질환으로 고혈압에 대하여 의사 진단, 진단 시기, 현재 유병 여부, 치료 여부를 묻는 문항에 하나라도 “예” 라고 응답한 대상자를 고혈압 환자로 정의하였다. 당뇨병 가족력은 부모님 또는 형제 중에서 한명이라도 당뇨병이 있을 경우, 가족력이 있다고 정의하였다.

4) 당뇨병

당뇨병은 미국당뇨병학회에서 제시한 기준에 따라 공복혈당 126 mg/dL 이상 혹은 당화혈색소 6.5% 이상일 경우로 정의하였다[35]. 추가적으로 국민건강영양조사 자료 중 의사 진단을 받은 경우와 당뇨병 치료를 받고 있는 경우도 포함하여 분석하였다.

3 자료처리방법

연구 대상자의 일반적 특성을 파악하기 위해서 빈도분석 및 기술통계를 실시하였다. 악력에 따른 신체계측 및 당뇨병 관련 변인 차이를 알아보기 위해 남녀 각각 절대 및 상대 악력의 삼분위수에 따라 세 그룹으로 나누었다. 그룹 간의 대상자 특성의 차이를 보기 위해서 일원변량분석 (One-way ANOVA)과 연령을 통제한 공분산분석 (ANCOVA)을 실시하였고, 사후분석으로는 각각 Scheffe와 Bonferroni 검증을 하였다. 추가적으로, 연령에 따른 공복혈당 및 당화혈색소와 악력 간의 관계를 알아보기 위해 전체 대상자를 10세 단위로 나누고, 각 연령층 안에서 절대 악력과 상대 악력의 삼분위수에 따라 세 그룹으로 나누었다. 또한 전체 연령 및 각 연령대 별 절대 악력과 상대 악력에 따른 당뇨병 유병률을 추정하기 위해 로지스틱 회귀분석 (logistic regression analysis)을 실시하여 오즈비 (odd ratios, ORs)와 95% 신뢰구간 (confi-dence interval, CI)을 산출하였다. 로지스틱 회귀분석 모델을 구축하면서, Model 1은 연령, Model 2는 연령, 흡연, 음주, 수입, 교육수준, 근력 운동 참여 여부, 고혈압 그리고 당뇨병 가족력을 통제하였으며, Model 3은 Model 2에 추가적으로 체중을 통제하였다. 모든 분석은 SPSS-PC version 25.0을 이용하였으며, 가설검정을 위한 통계적 유의수준은 α= 0.05로 설정하였다.

연구결과

1 악력에 따른 대상자 특성

절대 악력에 따라 남녀 각각을 삼분위수로 나누었을 때의 대상자 특성은 Table 1과 같다. 절대 악력이 높은 삼분위에 속한 대상자에 비해 낮은 삼분위에 속한 대상자의 공복혈당 (남: 100.81±21.04 vs. 107.44±29.44, 여: 96.60±18.59 vs. 101.96±25.98 mg/dL)과 당화혈색소 (남: 5.62±0.69 vs. 5.91±0.98, 여: 5.58±0.65 vs. 5.83±0.84%)가 유의하게 높았으며, 각 그룹의 당뇨병 유병률은 남성에게서 9.8%, 15.7%, 그리고 24.7%, 여성에게서 7.5%, 10.7%, 그리고 19.5%로 악력이 낮을수록 증가하는 경향으로 나타났다. 하지만, 남녀 모두에게서 상위 절대 악력 그룹의 연령이 하위 절대 악력 그룹에 비해 통계적으로 유의하게 낮은 것을 고려하여 (남 43.1세 vs. 61.4세, 여 45.4세 vs. 60.0세) (p <.01), 절대 악력에 따라 나눈 세 그룹의 혈당과 당화혈색소간의 차이를 연령을 통제하고 비교한 결과, 통계적 유의성은 나타나지 않았다. 반면에, 연령을 통제했을 때에도 체중, 허리둘레, BMI, 흡연, 음주, 교육수준과 수입, 근력 운동 참여 여부, 고혈압 그리고 당뇨병 가족력은 그룹간 유의하게 차이가 나는 것으로 나타났다.

상대 악력에 따라 남녀 각각을 삼분위수로 나누었을 때의 대상자 특성은 Table 2와 같다. 상대 악력이 높은 삼분위에 속한 대상자에 비해 낮은 삼분위에 속한 대상자의 공복 혈당 (남: 99.11±21.10 vs. 108.52±29.15, 여: 93.92±16.47 vs. 104.66±28.89 mg/dL)과 당화혈색소 (남: 5.59±0.68 vs 5.93±0.99, 여: 5.50±0.59 vs. 5.90±0.89%)가 유의하게 높았으며, 각 그룹의 당뇨병 유병률은 남성에게서 9.1%, 16.2%, 그리고 24.0%, 여성에게서 5.1%, 9.8%, 그리고 21.2%로 상대 악력이 낮을수록 증가하는 경향으로 나타났다. 또한, 상대 악력이 낮은 삼분위에 속한 대상자들이 나이가 많고, 허리둘레가 넓고, 혈압이 높았으며, 근력 운동 참여 여부, 교육수준 및 월 수입이 낮은 것으로 나타났다 (p <.01).

2 연령에 따른 악력과 공복혈당 및 당화혈색소의 관계

Figure 1은 연령을 10세 단위로 나눈 6그룹의 각 그룹 안에서 악력에 따른 당뇨병 관련 변인을 비교한 결과이다. 남성의 경우 50세 이전 그룹에서 절대 악력과 공복혈당 간에 유의미한 관계가 나타나지 않았으나, 당화혈색소는 20대에서 악력 수준이 낮을수록 감소하는 정 상관관계로 나타났다. 하지만, 50세 이후부터 악력이 낮을수록 공복혈당과 당화혈색소가 증가하는 것으로 나타났다. 여성의 경우 19-29세 그룹과 30-39세 그룹에서 악력이 가장 높은 그룹에서 공복혈당과 당화혈색소가 통계적으로 유의하게 높은 것으로 나타났고, 40-49세에서는 중간 그룹이 당화혈색소가 낮은 것으로 나타났다. 이후 연령이 증가함에 따라 악력이 낮을수록 공복혈당과 당화혈색소가 증가하는 선 경향을 보였지만, 그룹 간의 통계적인 유의미한 차이는 없는 것으로 나타났다. Figure 2는 연령에 따른 상대 악력과 공복혈당 및 당화혈색소의 관계를 보여준 결과이다. 남성과 여성 모두 전 연령층에 걸쳐, 상대 악력이 가장 높은 그룹의 공복혈당 및 당화혈색소가 가장 낮은 것으로 나타났다.

3 악력과 당뇨병 유병률의 관계

절대 악력과 당뇨병 유병률의 관계를 알아보기 위해 전체 대상자를 남성과 여성으로 나누어 로지스틱 회귀분석을 실시한 결과는 각각 Table 3Table 4와 같다. 모든 연령층의 대상자를 포함하여 분석한 결 과, 절대 악력이 가장 높은 그룹에 비해 가장 낮은 그룹의 당뇨병 유병률이 남성과 여성에게서 각각 3.02배 (95% CI: 2.67-3.42)와 2.96배 (95% CI: 2.63-3.33) 통계적으로 유의하게 높은 것으로 나타났다. 연령과 (Model 1), 건강행태요인 및 사회경제요인을 통제하였을 때 (Model 2), 남성에게서는 절대 악력이 낮을수록 당뇨병 유병률이 유의하게 증가 하는 것으로 나타났으나 (ORs: 1.21, 95% CI: 1.04-1.42), 여성의 경우 절대 악력과 당뇨병 유병률은 관계가 없는 것으로 나타났다. 반면 체중을 추가하여 통제한 경우, 절대 악력이 가장 높은 그룹에 비해 중간 그룹과 가장 낮은 그룹의 남성에서 당뇨병 유병률이 1.36배 (95% CI: 1.17-1.58) 그리고 1.65배 (95% CI: 1.40-1.94), 여성에게서는 1.13배 (95% CI: 0.97-1.31)와 1.41배 (95% CI: 1.21-1.64) 높은 것으로 나타났다. 세분화된 연령 층 안에서 악력과 당뇨병 유병률 간의 관계를 분석한 결과, 체중을 보정하지 않고 연령과 건강행태 및 사회경제요인을 통제하였을 때 (Model 2), 남성에게서는 50대 미만, 여성의 경우 60대를 제외한 전 연령층에서 통계적으로 유의미한 관계가 없는 것으로 나타났다. 하지만 체중을 추가적으로 보정하였을 때 (Model 3), 남성은 40대부터 여성은 50대부터 절대 악력이 낮을수록 당뇨병 유병률이 높은 것으로 나타났다.

반면 상대 악력의 삼분위수를 기준으로 나눈 세 그룹 간의 비교 분석을 실시한 결과, 전체 연령의 대상자에서 상대 악력이 가장 높은 그룹에 비해 가장 낮은 상대 악력 그룹의 당뇨병 유병률이 남녀 각각 3.17배 (95% CI: 2.79-3.60)와 4.96배 (95% CI: 4.31-5.70) 통계적으로 유의하게 높은 것으로 나타났다 (Tables 5, 6). 마찬가지로 연령, 건강행태요인 및 사회경제요인들을 통제한 후에도 (Model 2), 전 연령 층에서 상대 악력이 감소함에 따라 당뇨병 유병률이 통계적으로 유의미하게 증가하는 것으로 나타났다. 연령 층에 따른 상대 악력과 당뇨병 유병률의 관계성은 남녀 모두 연령과 상관없이 상대 악력이 가장 높은 그룹에 비해 가장 낮은 그룹의 당뇨병 유병률이 통계적으로 유의하게 높은 것으로 나타났다.

논의

본 연구는 한국 성인 남녀에서 악력과 당뇨병 유병률 간의 관계성 분석을 통해 악력이 당뇨병의 위험인자로써 갖는 의미를 밝히고자 하였다. 또한 당뇨병 질환의 특성을 반영한 악력과 당뇨병 유병률의 관계성을 알아보기 위해 절대 악력과 상대 악력을 비교하였으며, 추가적으로 연령대 별로 다르게 나타나는 악력과 당뇨병 유병률의 관계성을 비교하기 위해 대상자를 10세 단위로 나누어 세부적으로 분석하였다. 악력 삼분위수에 따른 당뇨병 유병률을 분석한 결과, 남녀 모두에서 상대 악력과 절대 악력이 낮을수록 당뇨병 유병률이 유의하게 증가하는 것으로 나타났다. 또한 통제 변인을 모두 보정한 후에도 절대 악력과 상대 악력 모두 당뇨병 유병률과 통계적으로 유의미한 음의 상관관계가 있는 것으로 나타났다. 10세 단위로 세분화된 연령 층에 따라 분석한 결과, 상대 악력의 경우 전 연령대에서 악력과 당뇨병 유병률의 관계성이 나타났으며, 절대 악력의 경우 남성에서는 40대, 여성에서는 50대부터 통계적으로 유의미한 관계성이 나타났다.

당뇨병의 위험성이 있는 성인에게서 근력 감소와 인슐린 저항성의 관계는 잘 알려져 있는데[14,3638], 이는 당뇨병 유병률이 높은 중년 성인 또는 노인에게서 근력 저하가 당뇨병 발병의 위험요인이 될 수 있다는 가능성을 제시한다[39]. 본 연구에서는 남녀 모두에게서 악력이 낮을수록 당뇨병 유병률이 통계적으로 유의하게 높아지는 것을 볼 수 있었다. Leong et al. [40]은 17개 국가의 35세부터 70세까지 131,691명의 성인 남녀를 4년간 추적 관찰하여 절대 악력을 이용하여 당뇨병 발병 위험도를 조사하였는데, 그 결과 성별, 연령, 교육수준, 신체활동 수준, 흡연, 음주, BMI 등을 보정했을 때 절대 악력의 감소는 모든 원인으로 인한 사망, 심혈관계 질환 사망, 심근경색증 발병률 증가와 관계성이 있는 반면, 당뇨병 발병률 증가와는 통계적인 유의미한 관계성이 없었다고 보고하였다. 본 연구와 대상자 수, 악력 수준, 절대 악력 삼분위수에 따른 각 세 그룹의 당뇨병 유병률이 모두 비슷함에도 불구하고 다른 결과가 나타난 것은 보정 변수로 사용된 대상자의 특성이 두 연구 간에 차이가 있었기 때문인 것으로 판단된다. 한편, 상대 악력을 이용하여 악력과 당뇨병 유병률의 관계를 분석한 연구에서는 본 연구와 동일한 결과가 나타났다. 즉, 상대 악력이 낮을수록 당뇨병 유병률이 증가하는 것으로 나타났고 이는 성별, 연령, 음주, 흡연, 고혈압을 통제한 후에도 통계적으로 유의미한 관계가 있는 것으로 보고되었다[25,41,42].

선행연구에 따르면, 당뇨병이 있는 대상자의 악력은 당뇨병이 없는 대상자의 악력 보다 낮게 나타나는 동시에[43] 연령은 더 높은 것으로 나타났다[18]. 절대 악력을 기준으로 나누어진 그룹 안에서도 연령의 차이가 나타났다[40]. 본 연구에서 악력이 가장 높은 그룹의 남성과 여성의 평균 연령은 각각 43.1세와 45.4세, 악력이 가장 낮은 그룹의 남녀 평균 연령이 각각 61.4세와 60.0세인 것으로 나타났다. 이는 연령이 당뇨병 유무와 악력 수준에 미치는 영향력이 매우 크다는 의미로 판단된다. 선행연구에 따르면 악력에 따른 혈당, 당화혈색소, 그리고 당뇨병 유병률을 보고자 할 때 연령의 영향력을 제하기 위해서 연령을 통제하기도 하지만[44], 통계적으로 연령을 통제하는 것 만으로는 악력에 따른 혈당, 당화혈색소 그리고 당뇨병 유병률을 분석하기에 제한점이 있다고 판단된다. 본 연구에서는 전체 대상자를 10세 단위로 나누고, 각 연령 층 안에서 절대 악력과 상대 악력에 따라 세 그룹으로 나누어 악력과 당뇨병 유병률과의 관계를 분석하였다. 그 결과, 연령과 체중, 더불어 건강행태요인 및 사회경제요인을 통제하였을 때 (Model 3), 남성에게서는 40대 여성에게서는 50대부터 절대 악력 상위 삼분위에 대한 하위 삼분위의 당뇨병 유병률이 통계적으로 유의하게 높은 것으로 나타났다. 반면, 상대 악력의 경우 전 연령 층에서 상대 악력이 낮을수록 당뇨병 유병률이 높은 것으로 나타났으며, 연령 및 모든 변수를 통제 한 후 (Model 2)에도 통계적으로 유의미한 악력과 당뇨병 유병률의 음의 상관관계가 나타났다. 상대 악력은 대사증후군[1719,21,22,24], 심혈관계 질환[45,46], 비알콜성 지방간 질환[47], 고혈압[48] 등의 유병률과의 관계성이 밝혀지면서 질환의 발병 위험성을 예측하기 위한 위험 요인으로 규명되어 왔다. 또한 상대 악력과 질환 유병률의 관계성은 연령과 상관없이 명확하게 나타났다. 선행 연구 결과와 동일하게 본 연구에서도 상대 악력은 연령에 상관없이 당뇨병 유병률과 통계적으로 유의한 관계성이 있는 것으로 나타났다. 하지만 절대 악력의 경우 중년 및 노인 층에서만 절대 악력과 당뇨병 유병률이 통계적으로 유의미하게 나타났는데, 이는 50대 이후부터 절대 악력이 감소할수록 공복혈당 및 당화혈색소가 높아진다는 결과를 통해 뒷받침된다 (Figure 1). 결과를 종합하여 볼 때, 근육량 및 근력의 감소의 폭이 크고[49], 당뇨병 유병률이 빠르게 증가하는 중장년층에 있어서[31] 절대 악력의 감소는 당뇨병 위험 요인으로써 적용될 수 있다는 임상적 의미로 판단된다[50].

본 연구가 한국 성인을 대상으로 악력과 당뇨병 유병률의 관계성을 규명하고, 절대 악력과 상대 악력의 당뇨병 예측 위험인자로써의 가능성을 확보했다는 점에서 의의가 있지만, 몇 가지의 제한점을 가지고 있다. 첫째, 국민건강영양조사자료를 이용한 단면적 연구로서 악력과 당뇨병의 인과관계에 대해서는 검증할 수 없다. 둘째, 당뇨병 진단에 있어서 공복혈당 및 당화혈색소 외에 일부 변인은 직접 기입 방법을 사용하였기에 부정확한 진단이 포함될 가능성이 있다. 셋째, 국민건강영양조사에서는 당뇨병 진단 정의에 제1형과 제2형 구분 없기에 제1형 당뇨병 환자가 포함되어 있을 가능성이 있다. 하지만 한국 소아 청소년의 제1형 당뇨병 유병률이 10만명당 1.36명임을 고려하였을 때[51], 분석에 영향을 미치지 않았을 것으로 사료된다.

결론

본 연구는 국민건강영양조사 (2014-2018년) 자료를 이용하여 우리나라 19세 이상 성인에서 악력과 당뇨병 유병률의 관계성을 연령에 따라 어떠한 차이가 나는지 분석하였고, 악력이 당뇨병 유병률과의 관련성을 절대 악력과 상대 악력에 따라 각각 비교함으로써 악력의 사용 방법에 대해 조사하고자 하였다. 본 연구 결과를 토대로 간략히 정리하면, 연령에 따라 악력과 당뇨병 유병률의 관계성에 차이가 있기 때문에, 악력을 통한 당뇨병 유병률을 조사하는 연구에서는 대상자의 연령을 충분히 세분화하여 분석을 실시해야 한다고 사료된다. 또한 근력이 약화되고 당뇨병 발생 위험률이 증가하는 40세 이후 중년에게 있어서, 악력을 통해 당뇨병 위험성을 예측하고 당뇨병 발병에 대한 적절한 예방책을 간구하는데 도움이 될 것으로 판단된다.
